We are providers for Medicare, Medicaid, Culinary, AARP, HPN, Sierra Choice, Pacificare to name a few insurance companies. Call our office if your insurance is not listed to verify if we are providers. We are NOT providers for Nevada Care or Arizona Medicaid. Our office requires prior authorization. This may be obtained from your primary care physician.
